When the evaluation is complete, a classification and recommendations for each classification will be determined and recorded.
Important Documents for the Evaluation
- Court Purpose Driving Abstract: You will need to obtain a copy of your driving records from the Secretary of State facility on Dirksen Parkway. There is a fee for these documents.
- The Law Enforcement Sworn Report and/or Notice of Summary Suspension: You will need to bring the document that was given to you during your arrest.
